Patients bring this form to their family doctors because their accountant or financial advisor told them get it signed.

The problem is most people do NOT qualify.

Being disabled, or being on disability, is NOT the same as meeting the strict CRA criteria for the Disability Tax Credit.

This misunderstanding can often put a strain on the doctor-patient relationship. The patient wonders, “Why won’t my doctor just sign off on this form that gets me a tax refund??”
